We’d like to share with you an elder care issue that occurred over the holidays. The call came from an adult child who needed help with her elderly mother.
The mother had moved to an assisted living community in New Jersey, and when her daughter visited over the holidays, she realized just how unhappy her mother was. Mom missed her home in Virginia, her church, and she did not identify with the other residents in the assisted living community who were not at the same acuity level.
Mom and daughter agreed that a return to her home in Hampton Roads was in order, AND they also agreed that mom would need help.
Working with the family, our elder care services company was able to develop and implement a plan of care with an acceptable level of risk that emphasizes the Senior's independence and supports those areas in which she requires assistance.
| • | We first agreed that there would be COMPROMISE on behalf of the senior parent and the adult child. |
| • | We agreed that weekly nurse visits would take place through an agency handpicked by Care Connect, to review medications and vitals, and conduct general, medical wellness checks. |
| • | We agreed to restrict driving to within 10 miles of her home only, and only during the daytime. |
| • | We agreed to provide escort service to required, monthly psychiatric counseling appointments. |
| • | We agreed to Care Connect making monthly case management visits to make sure that the selected agency provider continues to provide the correct support and observe the home environment. |
| • | We agreed to set up installation of a stair lift. |
| • | We will serve as the local emergency contact service in the event the senior is transported to the hospital. |
| • | We will provide a detailed report to the adult child/power of attorney as needed. |
