The plan we make for you is specialized and based on your specific goals – whether that care is at home or in an assisted living facility, private care home, continuing care community, or nursing home.

• Helping you and your family get the best possible care for your loved one now and for the future, at each stage of your loved ones life -- until end of life.

• Offering more than simple recommendations. We ensure that your plan is executed and supported throughout your lifetime by providing asset protection, estate planning, care coordination, and care placement.

The Life Care Protection Plan is the cost-effective answer to ensure that you and your family have all the answers and support that is needed to avoid stress and costly mistakes while caring for a loved one.

• You pay a percentage of your assets at the time our firm is retain
       o On that anniversary date, an annual fee is paid each year.

• In return, our professionals will provide to you important services such as:
       o Help with certain legal documents/needs
       o Full care management - covering periodic hospitalizations
       o Management of in-home care
       o Help during crisis situations
       o Assistance with transitions across care settings (assisted living, board and care homes, nursing care, hospice).
       o Full assistance with Medicaid and VA pension filing, if applicable
       o Periodic review of assets, expenses, income and medical.
